Roles and Responsibilities:
Task & Responsibilities
- Assess physical fitness, mobility, and functional abilities using approved assessment tools and guidelines.
- Identify physical impairments and disabilities and document findings accurately.
- Flag cases requiring specialized care and recommend appropriate medical or rehabilitation referrals.
- Coordinate with clinical teams and referral facilities to ensure timely and effective case management.
- Maintain complete and accurate assessment records, reports, and referral documentation.
- Provide guidance to caregivers and beneficiaries on physical wellbeing and rehabilitation needs.
- Ensure compliance with organizational SOPs, safeguarding, and confidentiality standards.
